Our wish for you, your children and family is to achieve the goal of never needing dental work in the future. Our purpose comes from the derivation of the word “doctor.” It comes from the Latin word docere, which means “to show, to teach or to teach to know.” We help achieve this goal of “teaching” with regular wellness and hygiene visits. A goal that can eliminate gum disease and future dental work for lifetime. This goal is our sincerest wish for you, your children & family. Meet Dr. Inna Yakubov

Dr. Yakubov has been delivering outstanding dental care almost 20 years. She earned her Doctorate of Dental Surgery from New York University Dental School in 1995, where she graduated at the top 5% of her class with distinctions and honors. The doctor completed advanced dental general practice residency at LICH Long Island College Hospital as well as a mini residency in treatment of TMJ and Facial pain Dr. Yakubov chose a wonderful location and started her private dental practice in 2004 with the desire and commitment to provide superior dental care. Dr. Yakubov has had extensive continuing education in cosmetic dentistry, implants, occlusion, comprehensive restorative care (crowns, bridges and dentures), gum therapy, surgery and root canal therapy, oral sedation, Lumineers and Invisalign. She constantly completes extensive educational programs in all areas of dentistry to equip herself with the most recent technology and procedures in dentistry.

Financial Policy

Thank you for choosing us to provide your dental care. We consider it an honor to have been chosen by you to do so. Our philosophy in serving people is to be informative, honest, and forthright. This Financial Agreement is indicative of our respect for your right to know ahead of time what our expectations are in the area of finances in regards to your treatment. If you have any questions or concerns about our FInancial Agreement please do not hesitate to ask our office staff.

DENTAL INSURANCE: You should understand that limitation in your insurance coverage your insurance company does not limit your liability to the treating doctor. We will gladly file your claims and accept the assignment of dental insurance benefits provided you agree to the following:
You must provide us with an insurance card and driver’s license and all the information necessary to verify your coverage and file your claim.
Your insurance policy is a contract between you, your employer, and the insurance company. We are NOT a party to the contract. Our relationship is with you and not your insurance company.
You are responsible for our fees and not what your insurance company allows or considers “usual, customary, and seasonable” all of which vary from one company to another. We will adjust our fees to accommodate insurance fees when applicable.
Although we may estimate your insurance benefits we are not responsible for their accuracy.
Knowledge of benefits as well as benefit amounts, limitations, exclusions, waiting periods, etc. Is entirely YOUR responsibility. Receiving our services indicates your acceptance of the responsibility to pay regardless of our estimate.
All charges not paid by your insurance company are your responsibility regardless of the reason for nonpayment. Not all the services we provide are covered benefits. Benefits differ from one company to another. Fees for non-covered services, along with deductibles and copayments are due at the time of treatment.
This dental office cannot render services on the assumption that the Doctor's charges will be paid by an insurance company. If benefits cannot be determined for any reason you are responsible for the full-time service.
Since insurance companies are difficult to predict and payment by your insurance is not guaranteed, we require that patients wishing us to file their insurance claims keep a credit card on file with our office. Once your insurance claim has been processed and payment has been received. We will process your credit card for the remaining balance of the claim or the “patient’s portion.” Filing insurance claims requires this office to accept a delay of payment for services and we are willing to do this if we have your current credit card on file.

PAYMENT POLICY:

We accept cash, personal checks, debit and credit cards-Visa, MasterCard, and Discover. We offer Care Credit for convenient monthly payments.
After dental insurance has paid its portion, a statement is sent to the mailing address on record for the remaining balance. Payment is expected within 25 days of the statement date to avoid finance charges.
If the insurance company does not pay in full within 30 days, it will be your responsibility to pay the balance. Please note that four weeks following the filing of your claim, your credit card will be charged the remaining malice if your insurance company has not yet paid.
We do not file claims for medical insurance or more than one dental insurance company per patient. We do not accept responsibility for secondary insurance.

PATIENTS WITHOUT INSURANCE COVERAGE: We provide written estimates of fees, and payments are expected at each visit for services rendered.

FINANCIAL ARRANGEMENTS: As a condition of treatment by the doctor, financial agreements must be made in advance. This dental practice depends upon payment for the cost of their care and financial responsibility on the part of the patient must be determined before the treatment. All emergency services, or any dental services performed without previous financial agreements, must be paid for at the time services are performed.

MINOR PATIENTS: The parent or guardian accompanying the minor is responsible for full payment. Unaccompanied minors pay for the services at that time of service. The broken appointment clause applies to minors as well as adults.

FINANCE CHARGES AND COLLECTION FEES: Finances charges will be applied to all balances and paid within 60 days of the monthly billing date. A late charge of 1.5% on the balance then unpaid and owed will be assessed each month until paid. It is your responsibility to ensure your insurance company pays promptly so you can avoid finance charges. We understand temporary financial problems may affect the timely payment of your balance. IN those situations we encourage you to communicate to us any such problems immediately so we can assist you in the management of your account.

OVERDUE BALANCE: An accountant with an unpaid balance past 120 days will be sent to the collection agency. At the time, you will be responsible for all costs incurred in the collection of your debt.
CONSENT & AUTHORIZATION: I authorize dental treatment and agree to pay all related professional fees. Fees not covered by my dental insurance will be promptly paid upon notification from this office. I have read and understood this document in its entirety, outlining the office policies and financial policies of Dr.Inna Yakubov’s office. Without any reservations, I agree to abide by the policies outlined.

Thank you for reviewing our financial policy and consent for treatment. We make every effort to explain your costs and to avoid misunderstandings so that we can focus on your health care. If you have any questions, please ask. We are here to help you get the dentistry you want or need.

MISSED OR BROKEN APPOINTMENT POLICY: Although we know that unforeseen circumstances arise from time to time, patients need to honor their appointments so that you’re hygienist, doctor, our staff and other patients can rearrange their schedules.

Our cancellation policy states that, upon your first cancellation in less than 48hours of your scheduled appointment, we will inform you of our cancellation policy and no fees will be assessed as long as you reschedule your appointment promptly. After this, any cancellations made in less than 48 hours of the scheduled appointment will receive an assessed fee of $50.

I have read and understood the appointment cancellation policy of the practice and I agree to be bound by its terms listed above. I also understand and agree that such terms may be amended from time to time by the practice.
